Centralized patient reported outcomes are often controlled and limited. This leads to systematic biases in developing patient care.
Jenner is a platform to collect anonymized outcomes for vaccinations and store them into metadata in return for a NFT as a reward.

Motivation
We survive the pandemic of a lifetime! So let's celebrate COVID-19 vaccination and the Cardano network by doing something good for the public! In this Catalyst event, we propose a system for collecting patient reported outcomes built on Cardano blockchain. It is a decentralized and autonomous platform for the goodwill to donate their data to public for research purpose with the focus on privacy and security. This introduces a new type of systems we call decentralized autonomous patient report outcomes or DAPRO, an innovation much needed in our public health sector.

Survey question (No PII or any specific data that can identify a person)
1. Vaccine type: Pfizer | Moderna | Johnson & Johnson | etc.. (Note: use CVX code for storage efficiency)
2. Fever after 1st dose (within one week after vaccination

Note: Zip Codes may be too specific. There is an option to get only the first 3 digits of the zip codes for de-identification (https://www.hhs.gov/hipaa/for-professionals/privacy/special-topics/de-identification/index.html) but City and Country are sufficient to get both anonymity and meaningful data.
Metadata standard for storing patient reported outcomes (PRO)
We plan to propose a CIP in Github to store the patient reported outcomes similar to NFT metadata standard (721 label).
Reward
Once the patient reported outcomes are collected. The app returns a NFT token as a reward. This token will be named Fauci, which is a computer generated art inspired by structure and shape of the SARS-CoV-2 virus with different features depending on the answers and other random features similar to CryptoKitties or Spacebudz.
This NFT would not have any monetary value, at least not intended. It is intended as a token of goodwill. It can also serve as a trust token for future endeavors by other PRO projects.
User flow
1. User goes to the web app
2. Connect with a wallet (probably with web wallet like Yoroi if possible)
3. Fill out survey questions
4. Submit with transaction fee + NFT minting fee + small service fee
5. Plutus code stores the survey question in the metadata and mints a Fauci token and send it to the user's wallet address
6. User receives a NFT to the wallet
Researcher Flow
Researchers query the outcomes according to metadata standard for PRO from the Cardano blockchain / database. The weekly dataset can also be made available to download in CSV format via web app.
